DIAGNOSIS:
Pituitary Stalk Interruption Syndrome
The MRI images show non-visualization of the pituitary stalk with an ectopic location of the neurohypophysis and hypoplasia of the adenohypophysis with an empty sella. These findings are highly suggestive of the pituitary stalk interruption syndrome. This patient was proved to have panhypopituitarism.
- Hypopituitarism is due to partial or complete deficiency of pituitary hormones which may derive from either pituitary or hypothalamic disease. Pituitary dwarfism is proportionate delay in growth due to growth hormone deficiency.
- Pituitary gland development is a complex multistep process involving fusion of adenohypophysis (Rathke’pouch, ectodermal) with neurohypophysis (neuroectodermal origin), occurring in early embryogenesis. The etiology is usually unknown. One possible explanation is trauma; involving transection of the stalk during breech delivery leading to an ectopic posterior pituitary.
- Infrequently a ssociated with midline anomalies, such as septo-optic dysplasia, Chiari-1, medial deviation of the internal carotids, micropenis and single central incisor.
