Acute Ovarian Torsion
A massively enlarged and congested ovary is not enhancing on the post-contrast study, with only the stalk enhancing. Prominent follicles are also seen at the periphery. This combination of findings is highly suggestive of ovarian torsion.
Ovarian torsion is an acute abdominal emergency and needs early diagnosis and prompt treatment. Acute abdominal pain is a typical presentation.
he imaging findings vary with the stage of torsion. Though Doppler is a diagnostic imaging modality, the MRI is also an important diagnostic tool. In acute torsion, the ovary with be swollen with multiple follicles separated by swollen and edematous stroma. With hemorrhage and necrosis, torsion presents as a necrotic mass, which lacks enhancement. The most important finding in any stage is lack of enhancement of the ovary with a thick pedicle between the mass and uterus. Early diagnosis can help in prompt surgical intervention with salvage of the ovary
