Both short axis images show mid-myocardial enhancement in the septum on the delayed hyperenhanced inversion recovery (IR) images.
This is abnormal. Of the conditions mentioned above, arrhythmogenic right ventricular dysplasia predominantly shows enhancement in the right ventricular walls, which is absent. Infarction always shows subendocardial enhancement.
Mid-myocardial enhancement typically occurs with infiltrative or inflammatory cardiomyopathies and reflects the presence of inflammation, infection or fibrosis. Sarcoidosis is a typical cardiomyopathy that presents in this manner.
This patient had presented with supraventricular tachycardia and also had non-necrotic bilateral hilar nodes.
